Female adult hormonal acne is primarily caused by fluctuations in hormone levels, especially androgens like testosterone, DHT or androstenedione, or steroid hormones like cortisol which stimulate oil production in the skin’s sebaceous glands. These hormonal imbalances can be triggered by factors such as menstrual cycles, stress, hormonal disorders (like polycystic ovary syndrome), and lifestyle influences. Increased sebum production combined with clogged pores creates an ideal environment for acne to develop. Additionally, sensitivity of the skin to hormonal changes and inflammation can exacerbate the condition.

Cortisol, commonly known as the stress hormone, can significantly impact female adult hormonal acne. Elevated cortisol levels due to chronic stress stimulate the adrenal glands to produce more androgens, which in turn increase oil production in the skin. Additionally, high cortisol levels can promote inflammation and impair the skin’s ability to heal, making existing acne worse and prolonging recovery.


Insulin resistance can contribute to female hormonal acne by disrupting hormonal balance. When the body's cells become less responsive to insulin, the pancreas produces more insulin to compensate. Elevated insulin levels can increase androgen production, which stimulates excessive oil production and clogging of pores. This hormonal imbalance promotes the development of acne, particularly in adult women. Managing insulin sensitivity through diet, exercise, and lifestyle changes can help reduce hormonal fluctuations and improve acne symptoms.
